The JW Marriott Marquis Hotel Dubai will help submit and process visa applications for your upcoming stay in the United Arab Emirates and to attend the IHSI Conference.

1- Completed visa application form provided by the hotel (to be signed by the passport holder): Click Here for Visa Application
2- Clear scanned copies of the first pages of your passport (.jpg format and full color)
3- Clear scanned copy of the applicant’s passport picture with white back ground(.jpg format)
4- Completed credit card authorization letter which is provided by the hotel. Form must be signed by the card holder: Click Here for Card Authorization

About the Dubai Department of Tourism and Commerce Marketing (DTCM) Requirements: Local Dubai legislation applied to all events in Dubai including the iHSI Meeting require additional information regarding attendees:

Participant information: All attendees are required to supply the following details via the meeting registration form: Full name, Country of residence, Date of birth, Nationality, Mobile phone number and Email address. This information will be passed on to the DTCM, who will use it to create barcodes to be displayed on attendee meeting badges. As an attendee, you need to wear your badge at all times throughout the meeting, including the social events.

Speakers’ details: In accordance with requirements from the DTCM, all those who appear on the event agenda as speakers will be asked to upload, via a secure webform provided by the iHSI, the following details: Passport number, Date of birth, A short biography (at least 60 words), If resident of UAE, an uploaded copy of their Emirates ID (in jpeg format). The iHSI will pass this data on to the DTCM. The data will be securely handeled and permanently deleted one week after the iHSI Conference Further information about DTCM available online.
